I have heard several elders talk about 'pechotti chatkigayche' when someone gets severe back pain, sometimes making the person immobile from the pain.

Usually the pechotti is fixed by a physical maneuver involving the toe. The patient instantly feels the relief.

Does medical science have an explanation for this?

Ws.

From what I understand, "pechotti" is more like muscular spasm that one feels on the back leading to intense pain and immobility. Usually the nerves that reach the foot and toe arise from your back.

From my research, medical science does not have a clear-cut explanation for the above treatment method. But I can imagine it has to do with something related to the connection that the toe has with the spine through the nerves.
